Kaiser Permanente recently launched the Kaiser Permanente Community Health Needs Assessment (KP CHNA) Hub on Community Commons. Formally a stand-alone site, the KP CHNA Data Platform was moved to Community Commons earlier this year.
With updated data, refined indicators, and a new Preliminary Health Needs Identification Tool (PHNIT), the KP CHNA Data Platform was specifically re-designed and updated for the Kaiser Permanente hospital system community benefit reporting process.

Create a CHNA Report using KP-Identified Indicators
Built using the Community Commons’ public CHNA report as a foundation, the KP CHNA report includes additional indicators and a variety of report options, including:

- In-Depth Potential Health Needs Report

Check Out the Preliminary Health Needs Identification Tool
The Preliminary Health Need Identification Tool is the most recent addition to the KP CHNA Data Platform. The purpose of the tool is to provide quantitative analysis of secondary data to identify potential health needs in a community.
The goal of this tool is to make the secondary data analysis process more efficient for users, to provide a defensible scoring method for identifying and prioritizing potential health needs, and to provide a benchmarking option for your area of interest.
